The unit refereed to above was the same unit which Mr. Pickett noticed the dropped ballot in South Carolina.  I have isolated the problem to the reader.  The reader seems fine in all other respects.  Further troubleshooting will tell us more. 
We have yet to see this occur in another unit though we have not been watching.  Will add a procedure to the final test of the tabulators to see how frequently this event occurs.

    What is the Reader firmware level in the Reader and the Accu-Vote?

The firmware of the reader in question is 12.5.  Thank you.  I did not think of reader firmware as the cause.
The unit somehow got past us.
Everyone should be aware that everything works correctly with 13.5 or newer firmware.

Also for everybody's piece of mind, know that all visible light readers have been loaded with 13.5 or newer firmware since 1996 and previously made readers were retro-fitted with the 13.5 firmware.  It appears that there was a single 12.5 straggler in Pickett's unit.
