This is not so much a bug as an unimplemented feature.  There is an outstanding RCR or two on the topic.  One big gotcha in this requirement is that there might be more than one device creating challenge cards.  Storing the list of ids created on one machine doesn’t solve the problem.  There are ways to deal with that (both good and bad), but none are implemented and no conclusive design is in place.  File this under “the way it works” until the whole project of “challenge voter processing” gets taken on.

When creating voter cards - if you specify that it is a challenged voter, the system does not require you to enter a unique category id for each voter card within the precinct.
