That's exactly what the printer has done, reversed the images.

Hmmm, this could be interesting...  I printed out your artwork on my HP and it reads fine. 
 
I bet you printed the back plate "upside down".  Hold card in portrait orientation front side up.  Note the id marks are at the bottom.  Now turn it over flipping along the long side.  Still holding it in portrait, are the id marks still at the bottom?  If not, that is the problem.  You can compare against a standard portrait ballot for reference if the above explanation is confusing.
 
Holding the ballot my first instinct was to flip it short side instead of long.  I'll rotate the back image 180 degrees.
